Approaching a highly sentimental trip with long time friends, our patient was fearful that her pain would put a halt to their 30 year tradition. Some of her friends had passed throughout the years; and it was getting more challenging to take these coveted trips together.

After an in-office X-ray, we discovered a stress fracture in her 3rd metatarsal (top + middle of her foot). With only 5 weeks until the trip, you could see the devastation through her strong façade. Who is NOT a candidate?

Patients with active cancer, those who are pregnant, have a dark pigmented lesion underneath the nail (this requires a doctor visit first for proper evaluation), or have severe injection around the treated area are not candidates for laser therapy.

Who must see a provider before laser therapy can be started?

Those who have a dark pigmented lesion underneath the nail, diabetes, peripheral neuropathy, peripheral vascular disease, arterial disease, or post-surgical patients.

The above underlying conditions require a pre-screen to take additional precautions to ensure the best possible outcome. (You may use your insurance for any doctor visit)

Post-surgical patients need an order from your doctor.

Is this covered by my insurance?

Dr. Brooks will deliver same day preliminary results and reports, which means we are able to put the power of your treatment plan in your hands right away.

We’re very fortunate to offer this specialty testing at Lexington Podiatry. He has been performing this highly specific studies in our office for over a decade. Admired in his field, he frequently travels to train and educate his colleagues. He is the top expert in his field on nerve damage, specifically lower extremity, although he is well versed in upper extremity neurological issues as well. His services are generally covered by insurance and as a courtesy, we will verify with your insurance before your coveted appointment with him.

Is laser therapy the right decision for me?

Laser is a great option for people from all walks of life! Laser is safe for children + elders, non-invasive, offers rapid relief with no down time, and is FDA-cleared. Unlike other treatment options, with laser therapy you don’t have to worry about painful blood testing, nail biopsies, or harsh medications.

Laser therapy is a modern, safe treatment option to eliminate fungus and prevent reinfection. Laser can also treat thick nails from trauma from a direct injury, or after a surgical procedure like an ingrown removal.

Is this covered by my insurance?

Unlike harsh oral medications, laser therapy may be considered ‘cosmetic’ by your insurance company. While the MLS Therapy Laser is FDA-cleared, treatments aren’t covered by insurance for nail fungus. If you have FSA, Health Savings Accounts (FSA) or a Medical IRA the cost can be covered.

We do encourage you to coordinate directly with your insurance company for approval prior to your appointment. This will allow our Patient Experience Team to potentially get prior approval and allow your visit to go seamlessly.

With rising appointment fees and out of pocket co-pays, the cost of the laser therapy treatments is actually less costly than other, more invasive treatments that are “covered” by insurance.

Is it FDA cleared?

Yes, it is FDA cleared for treating patients who have onychomycosis (nail fungus).

What is toenail fungus?

Toenail fungus is an infection of the nail that can present in various ways. From small white dots, to thick patches of discoloration, and even crumbly nails. Without treatment, this fungal infection can spread to other toenails very quickly. The sooner you start treatment, the sooner you’ll have healthy nails.

My nail fungus keeps coming back!

This is common for a lot of us! Fungus is difficult to treat because it can get into the layers of your toenail or even grow underneath the nail. Trying to remove this at home is tedious, and can even lead to an ingrown toenail or painfully thick toenails. Without treatment fungus can also spread to other parts of your body.

What causes nail fungus?

The culprit: dermatophytes. This is a group of fungi that cause superficial infections and thrive on keratin for growth - hence the nail fungus. Fungus doesn’t always come from the locker room, any warm, moist environment is ideal for fungus. From pools + hot tubs to dirty socks + shoes, fungus can be hard to avoid.

If you sweat excessively or wear non-breathable shoes for work, you’re more susceptible to develop nail fungus. So are people with diabetes or weakened immune systems, as circulation problems give fungi room to roam free.

What if I have unsightly nails, but no fungus?

Laser therapy is effective on all nail damage, not just fungus. It is not uncommon for those who have had some type of trauma to experience nail discoloration or thickening. If you’ve been tested and are negative for fungus, laser therapy is a viable option for improving your nail health + appearance.

Does it really work?

Absolutely, with the right candidate! Dermatologists have used laser therapy to treat nail fungus and unsightly nails for decades. Laser therapy has been approved by the FDA since 2009 and is clinically proven to be up to 93% effective after multiple sessions. In our office the average is 6 sessions.

Current oral antifungal prescription options are <50% effective, with prescription topicals being only 16% effective.

Laser treatment works by heating nail layers, successfully penetratingly to reach the fungus, and attempting to destroy the genetic material responsible for fungus. This method of delivery makes laser therapy substantially more effective than topicals.

Clinical research supporting laser therapy

Backed by multiple clinical studies, laser therapy is an FDA approved treatment for nail fungus. A 2016 study found that patients displayed clear nails after 3-6 months. Further research from 2019 published by the American Association of Family Physicians yielded remarkable results along with greater effectiveness than over-the-counter medications: 90% reduction in symptoms and 30% total cure rate within 180 days!

The Lasers In Medical Science Journal also reported success rates among patient populations infected on two thirds or more of their nails at 83%. Nail fungus can be treated without concern thanks to modern technology!

How does it work?

Laser therapy treats nail fungus through targeted heat application. The frequency of the laser reaches fungus below the nail surface, effectively destroying the fungi’s ability to grow and reproduce.

The heat laser breaks down infected tissue, destroying the fungus as well as the surrounding skin + nails. The heat from the lasers also has a sterilizing effect, which helps prevent new fungal growth.

Depending on the intensity of your condition, the infection can partially survive the treatment. Due to this, it's unlikely that you will be completely cured of toenail fungus with just one session of laser therapy. However, after a series of recommended laser treatments has been completed, you will notice a significant improvement in the appearance of your nails.

Laser treatments for toenail fungus are considered safe but may cause superficial irritation if you’re particularly prone to heat sensitivity.

Can I treat my nails with over the counter products?

Over-the counter products may offer temporary relief for superficial infections only; however, they often lack the necessary strength needed for complete removal of these fungi.

Laser Therapy vs. Oral Medication

Laser therapy is SAFE for you!

A lot of our patients aren’t even candidates for the oral medication: those with poor circulation, medications (cholesterol medications, etc.), debilitating diseases, auto-immune issues and/or liver issues.

Bonus: Laser treatments are typically WAY more effective than other treatments for nail fungus (93% vs. 49%)!

“I left my polish on too long, it’s not fungus!

It is. Superficial fungal nail infections (called Superficial White Onychomycosis, “SWO”) are often mistaken by many patients who think it’s from leaving their nail polish on too long.

Make no mistake, this is the start of a nail fungal infection. Sometimes surface fungal infections can be treated with REPAIR. Our doctor-created blend of 2 potent antifungals (Rx: clotrimazole + tea tree oil), Urea (idea for thick nails) + proprietary blend of hydrating oils to allow for supple cuticles while strengthening weak nails.

I think my nails are a lost cause.

Don’t give up on yourself.

If you’ve tried treatment after treatment with no success, it is likely because the nail is “dead” or embedded into the nail bed itself.

If the nail is completely detached, entirely white, or too thick for the laser to penetrate, the nail must first be gently removed by your provider. This is a provider visit and can be billed to your insurance if you prefer. Before we’ve scared you off…most patients are able to start therapy without nail removal.

It’s been all over the news that antifungals are extremely harsh on the body, and it can cause full-body (systemic) side effects, specifically liver damage. Lastly, oral medication is LESS EFFECTIVE than laser therapy, and does not necessarily cure toenail fungus. The range of success for oral medication is anywhere from 40%-81%.

Prescription topical treatments alone are generally considered safe but are only 16% effective.

I’ve tried laser therapy and it didn’t work.

This could be due to a few different factors.

Undiagnosed Condition

Contrary to popular belief, fungus is not necessarily a hygiene issue. Most of our patients have undiagnosed Raynaud’s Disease or another circulation issue that has been untreated thus not responding to other laser treatments.

Incorrect Laser

Lexington Podiatry is one of the only medical practices that have lasers SPECIFICALLY USED TO TREAT FUNGUS + TRAUMA.

Other clinics are using lasers that are multi-purpose for cosmetic skin issues. Our laser is FDA-cleared specifically for nail fungus + trauma, allowing us to achieve results other clinics cannot.

What to expect during your visit

You should remove all nail polish from your nails before your appointment. We recommend only wearing nail polish during special occasions for short periods for optimal nail health.

FIRST your laser clinician will perform an extensive exam and medical history review to ensure your treatment plan is tailored for your success.

If necessary, you may have your toenails clipped and dead skin cleaned (debridement) on the day of your laser treatment. We encourage you to get to do this at home a few days before your visit and can help offer guidance on how best to accomplish this.

NEXT, the laser will be moved over your affected nails. It may also be used over non-affected nails if there is a concern of fungus spreading

Using selected wavelengths helps minimize heat on the skin, which reduces the risk of side effects. One session of laser therapy typically lasts 10 minutes or less based on the number of nails.

As the tissue breaks down, you may have minimal discomfort or a small blister may develop. Although this is extremely rare, rest assured the skin will heal in a few days. Your healthcare provider will tell you how to keep your toe clean and dry as it is healing.

The treatment is almost always painless, though you may feel warmth or tingling on the nail bed during the treatment. Typically, the warmth feels good to those with cold toes.

My nails don’t grow.

Nail growth is directly dependent upon blood flow. So if you suffer from poor circulation, we encourage you to schedule a visit with our providers prior to your laser therapy visit For your convenience, this can be done on the same day.

At this visit, your doctor will prescribe topical medication to improve your circulation, as well as offer many helpful tips to promote blood flow. Keep in mind: if your toes are cold, your nails aren’t growing. We must address this to give you the best possible chance of getting rid of those unsightly nails.

What can I expect after treatment? Do I have to stay off of my feet?

You could literally go run a marathon after your laser treatment. There is no downtime or restrictions after your treatment.

Though it can take six months to a year for your nail to return to its former appearance (depending on the extent of the infection), patients will typically notice healthy new nail growth without damage to the surrounding tissue.

The laser treatment can prevent the fungus from spreading and reduce the pain or discomfort associated with infection. While recurrence of infection is unlikely, some fungal infections may return and require additional treatment.

How do I know if it’s working?

With perfect circulation our nails grow very slowly, approximately 1 mm per month. You won’t notice the fungus being eliminated overnight. In fact, even effective treatments are not expected to clear an infected nail immediately—it may take up to 12 months until the nail grows out. However, you should see clearing around the cuticle indicating a new, healthy, fungal-free nail is growing out!

Signs of improvement in toenail fungus after laser therapy include:

  • The disappearance of yellow, white, or brown discoloration
  • Decreased flaky debris on or under nail
  • Reattachment of the nail
  • Normal shape + texture restored

Will my nail fungus come back?

During your visits our Laser Clinician will discuss with you your lifestyle, work environment, contributing medical conditions, and any other triggering events that are causing you to get recurrent infections.

With your help, we cultivate a truly customized plan to make sure we minimize elements that are triggering fungal infections.

Can I laser my finger nails?

YES! You can laser not only all fingernails + toenails, it is safe for skin as well!

Fungal infections on the skin, such as athletes’ feet, respond very well to laser therapy. Isolated fungal rashes, unresponsive to other topical and oral treatment options prove very responsive to laser therapy.

The number of visits for skin fungal infections is dependent upon the size of the area, and responsiveness to the therapy. We encourage a provider’s evaluation prior to treating fungal infections on the skin.

Can I get a pedicure?

As an all-female group of providers, we get it…we like our toenails painted too! However, as you’re making this commitment to get rid of unsightly nails it is best to GO NAKED! Our prescription-strength topical, called REPAIR will be applied daily as part of your foot care routine. REPAIR is actively killing fungus, preventing build up and minimizing your risk of it spreading to other toenails. However, we like to look good too!

If you have a special event, it’s safe to paint your nails. We offer Dr. Remedy’s Anti-fungal Nail Polish in our office. This nail polish is breathable and has tea tree oil which is nature’s natural anti-fungal.

We often get fungus from leaving nail color on because it’s too thick and doesn’t allow the nail plate to breathe. This traps any fungal spores on the nail plate and allows additional growth of fungus under the color. EEK!

We recommend not keeping it on too long, it is important to remove all nail polish the day before your treatment. Following the treatment, polish can be applied immediately.

Who is NOT a candidate?

Patients with active cancer, those who are pregnant, have a dark pigmented lesion underneath the nail (this requires a doctor visit first for proper evaluation), or have severe injection around the treated area are not candidates for laser therapy.

Who must see a provider before laser therapy can be started?

Those who have a dark pigmented lesion underneath the nail, diabetes, peripheral neuropathy, peripheral vascular disease, arterial disease, or post-surgical patients.

The above underlying conditions require a pre-screen to take additional precautions to ensure the best possible outcome. (You may use your insurance for any doctor visit)

Post-surgical patients need an order from your doctor.

How do I talk to a nurse?

Our phone number is . Due to the high volume of calls we receive daily, we use an automated system for all phone calls. Voicemails are checked several times throughout the day and discussed with physicians in between patients. We will make every effort to return your call by the end of the day. Routine calls received after 4:30 may not receive a phone call till the following day. Urgent calls are triaged by our nursing staff and calls are returned as quickly as possible.

At any time, you may contact your provider securely through the patient portal and they will respond within a day, during normal business hours.

Where Do I Park?

We are located just off Sir Barton Dr. in the Hamburg Shopping Area with handicap accessibility, plenty of parking directly around the building and easy access from the interstate. What do I need to bring to my new patient appointment?

  • If you haven’t already, please fill out the new patient information through our patient portal. If you did not receive an email with your login information, please call 859-264-1141.
  • Please read over our Privacy Policy and Financial Policy.
  • Please bring your driver’s license, valid insurance card and form of payment for your co-pay. Co-pays are due before service.
  • Make sure your insurance information is valid. Outdated information may result in claim denials by your insurance so be sure they are accurate. You will be responsible for payment of any claim denials.
  • Please wear or bring the shoes you wear most often. If you may be fitted with a brace, bring a pair of tennis shoes.
  • If you have orthotics, please bring them – even if they are from somewhere else or if you didn’t wear them much.
  • Bring a list of the medication you are currently taking.
  • If you will need assistance getting into our offices, please let us know ahead of time. We’ll be happy to help! Please arrive a little earlier than your appointment time to get settled in if you need assistance.
  • In order to ensure we are respectful of other patients’ scheduled appointments, we will reschedule your appointment if you are more than 15 minutes late.
  • Minors (under age 18) should be accompanied by an adult with legal medical-decision-making authority during their initial visit. On subsequent visits, the minor must also be accompanied for treatment by an adult. However, many times legal guardians are not able to accompany their teen or young adult child to an appointment. Please call the office for a form to designate individual authority to consent to any and all treatments.

What should I know about my surgical date?

Day Before Your Surgery

  • Explicitly follow any guidelines given to you at your previous visit.
  • Don’t shave near the area where the surgery is to be performed.
  • If you are a smoker, please don’t smoke the day before.
  • Don’t eat or drink after midnight.

Day Of Your Surgery

  • Make sure you arrive an hour before your appointment time at the Kentucky Surgery Center and Baptist Physicians Surgical Center or two hours before your appointment time at St. Joe East.
  • If you’ve been instructed to take any routine or other medication, please take it prior to showing up for your appointment.
  • Wear loose and comfortable clothing, including flat shoes.
  • If you wear contacts, please wear your glasses instead.
  • Depending on the surgical area, or if you are administered general anesthesia, you will not be permitted to drive home. Make sure you’ve made arrangements.

Do you run on time?

We are very mindful that you have many choices for medical care and we thank you for choosing us. Our providers are attentive to the individual needs of each patient. Appointments most often run on time, but if there is a medical emergency, an appointment time may be delayed. In the rare event that we are delayed, we will do our best to communicate in a timely manner and assure you that you will receive the same individualized care.

Please help yourself to the free coffee and hot tea in our reception area. Additional snacks are available should you need some refreshment while you wait. Just ask our friendly receptionist team!

We strive to run on time to respect our patient's busy schedules. If you are late for your appointment time, we will do our best to see you in order, but you may be worked in behind other patients that were on time. If you are 15 minutes late, you will be rescheduled.

How much will my visit cost?

The cost of your visit depends on your insurance. Many times, the front of your insurance card will tell you the co-pay that you will be responsible for paying when you come to see a specialist.

Your co-pay is due before your service.

However, as a courtesy to you, we will call your insurance company ahead of time to see if your first visit will be covered and to get an estimated coverage for various services that we offer (which you will be informed of upon your first visit).

Have a high deductible?

Don't worry, we understand. Please contact our Billing Manager at ext 53 if you are interested in learning how you can save by becoming a self-pay patient at Lexington Podiatry OR examine some of our most common self-pay treatment prices.

We care about your health, and want to make sure you have options to get the help you need. Many treatments and medical products are covered by Health Savings Accounts and Flexible Spending Accounts. In addition, we offer Care Credit.

Will my X-Rays be covered by insurance?

Depending on your insurance, X-Rays are typically a covered treatment by most insurance plans. If your insurance happens to not cover them, we can work with you to set a prompt pay price, usually running between $30-$50.

How quickly can I return to work after my ingrown toenail procedure/removal?

We recommend taking it easy the rest of the day.

If you must return to work that day and have an office position, prop your foot up while wearing an open-toe style shoe to avoid putting pressure on the nail.

If your position requires some type of steel-toe or rigid shoes and you must return to work, we recommend taking it 'as easy as possible' and frequently remove your shoe to visually inspect for any signs of bleeding or swelling.

This is not recommended as continuous pressure immediately following removal can delay healing and increase chances of an infection.

What does a nerve biopsy show?

This test determines if the small nerve fibers throughout your legs and feet are functioning properly.

Depending on if you're a male/female and your age, you have a certain range that your nerves should be functioning at. This test shows if you are functioning in your range or not and measures the density of the nerve fibers found in your skin. From there, we can determine if you need a medication or need to be referred out to a neurologist for more testing.

Will insurance cover DME (durable medical equipment)?

We provide complimentary insurance verifications to check benefits/DME coverage before your appointment.

If you have a deductible that has not been met, you may be required to cover the portion that your insurance doesn't cover. We do offer Care Credit and payment plans on certain items. Our billing department can help clarify your questions, but your insurance provider is a valuable resource for these questions as well.

How many pairs of orthotics does my insurance cover?

Many insurance plans cover one pair of orthotics per year, depending on the diagnosis. Some insurances may actually cover two per year. We provide a complimentary check of estimated benefits before your visit to reduce the chance for surprises.

Are the orthotics a permanent fix for my issue?

No. However, it is the most effective prescription corrective device used to prevent your issue from getting worse.

Do I need to remove my toenail polish when visiting the podiatrist?

Yes. Your toenails reveal many things about your overall health, including whether you may have toenail fungus.

Is laser therapy for fungal toenails covered by insurance?

No. Although it is the most advanced treatment for fungal toenails, laser therapy is still considered a cosmetic service. Many lasers that are being advertised are not strong enough to penetrate the nail bed - it's necessary to use a laser that is strong enough to reach the fungal infection. Although insurance does not currently cover laser therapy, Care Credit, Health Savings Accounts and Flex Spending Accounts will cover the procedure. Does a minor have to be accompanied by a parent?

Minors (under age 18) must be accompanied by an adult with legal medical decision-making authority.

On subsequent visits, the minor must also be accompanied for treatment by an adult. However, many times legal guardians are not able to accompany their teen or young adult child to an appointment.

Please contact the office for a Consent to Treat form to give the designated individual authority to consent to any and all treatments.

What is the process for FMLA paperwork?

You may bring in, fax, email or send the paperwork through the patient portal.

There will be a $25 charge for every FMLA paperwork request.

Do you offer free foot exams?

Yes! We often take our medical team to corporate events, job sites and health fairs to conduct complimentary foot health screenings.
